What is VA Special Monthly Compensation?

VA special monthly compensation is an additional tax-free benefit paid to veterans, their spouses, surviving spouses, or parents who have especially severe service-connected disabilities. This compensation is awarded in cases where a veteran has lost, or lost the use of, limbs, organs, or senses such as arms, legs, or eyesight, or when they require aid and full supervision, meaning help with daily activities like bathing, dressing, or eating. Veterans who are housebound due to service-connected conditions may also qualify. SMC is intended to recognize and support veterans whose disabilities go beyond the standard VA disability rating system.

Do you Qualify For SMC?

The VA Special monthly compensation is for:

  • The loss or loss of function of specific body parts, such as the arms, legs, or eyes
  • Suffers from severe disabilities that make daily living more difficult
  • You require regular aid and attendance from another person to perform daily activities like bathing, dressing, or eating.
  • You are housebound due to your service-connected condition.

Levels of SMC:

Each SMC level is designed to account for the extent to which a veteran’s disabilities affect their ability to live independently. The more severe the disability or the greater the need for aid, the higher the level of compensation.

  • SMC-K-  A small additional payment for the loss or loss of use of specific body parts or a creative organ. It can be added to other SMC levels.
  • SMC-L to SMC-O- These levels cover more serious conditions, such as the need for regular aid and attendance, being housebound, or the loss of multiple limbs or senses. As the letter level increases, so does the compensation.
  • SMC-S- Given to veterans who are 100% disabled due to one condition and have additional disabilities rated at 60% or more, or are permanently housebound.
  • SMC-R-Awarded to veterans who qualify for aid and attendance and require a higher level of personal care, such as assistance with nearly all daily activities or continuous supervision.
  • SMC-T- Reserved for veterans with traumatic brain injuries (TBI) who require ongoing, daily care equivalent to nursing home-level support.

How To Apply

  1. Complete and submit VA Form 21-2680. This form provides detailed information on your daily living activities and the assistance you require.
  2. Gather all your medical information.
  3. You can submit your application by mail, in person, or online. 
  4. The VA will review your application and medical evidence.
  5. You will receive the VA’s decision letter regarding whether you are eligible for the SMC and how much compensation you can receive.
  6. C&P exam (If needed), VA may schedule a compensation and pension exam to further assess your conditions and to see if you are eligible
